Injury Reporting & Risk Management
Incidents and injuries must be recorded on the HC Injury Report form and submitted to the Tri Port Administrator, who in turn will report to BC Hockey. Below is a memo from BC Hockey about the requirements of injury reporting as well as a link for the Hockey Canada Injury Report form.
