Tri Port Complaints Form


As per Triport's MHA Policy manual, if a parent, player, volunteer or other member has a significant issue/complaint that can't be resolved at the Coach/Head Coach level, then please identify the issue and explain in detail using the online form below.   The complaint will be forwarded to the Tri Port President for investigation.    The President may appoint designates, form committees or defer matter back to Club Executive for resolution.
